The Vial of Life can potentially save you or a loved one's life by providing crucial information to Emergency Medical Services and the hospital, especially when the person is unable to communicate. Simply fill out the form, put it in an envelope, and place it in your refrigerator. A sticker is provided to place on your door or refrigerator to alert emergency personnel. This information will be accessed by EMS when they respond to an emergency at your residence.

The VIAL OF LIFE kit can be picked up during normal business hours at Station 112 located at 64279 Hwy 3081, Pearl River, LA 70452.
