Please print the application, background check, and criminal history form. After filling them out, kindly submit them to Station 112, located at 64279 Highway 3081 Pearl River, LA 70452. If you are unable to open or print a .pdf file, please feel free to email us at contact@pearlriverfire.org.

 

For the background and criminal history forms, please fill out your basic information at the bottom. Additional information will be completed by fire department personnel.

 

The L4 and W4 Forms are for tax purposes and can be completed upon being hired.

 

OSE Competitive and Promotional applications are required for the respective type of application and testing, but they do not need to be submitted at the time of the application, background check, and criminal history submittal.
